Under Integra ServiceConnect we do not attempt to treat members into better health service utilization. We help them avoid situations that can trigger admissions while facilitating access to more appropriate levels of care. Our focus is on identifying and modifying day-to-day psychosocial factors that encourage over-reliance on emergency rooms and hospitals when care is sought. In addition to working closely with members, Service Coordinators actively enlist the support of friends, family, clergy and health plan staff at every step along the way. Equipped with Tablet-based remote communications technology, our team is fully functional in the field, supported by real-time reporting, resource consultation and event tracking using a state-of-the-art web-based electronic health record.
Integra ServiceConnect also offers an alternative to Fee-for-Service compensation that incentivizes maximum billing and is common in traditional provider-based case management. Reflecting our deep managed care history and experience, we align our incentives directly with our clients' own financial and performance objectives, including risk/reward sharing arrangements under which performance and outcomes (e.g., HEDIS) determine our ultimate compensation.
