Integra ServiceConnect is designed to complement existing clinical case management and medical management programs. In most cases health plans already provide these services and deliver them well. Our approach provides clinical supervision and management mentoring to a team of well-trained and well-coached community service coordinators recruited from the same communities as the members we manage. The Integra ServiceConnect team works directly with members in their community, minimizing admissions and helping payers meet important quality and fiscal goals.

Integra ServiceConnect is developing a strong track record of performance, bringing value to health plan clients in two critical areas:

• Increasing HEDIS scores impacted by high-risk, high-cost members
• Managing total costs for these members, while improving the quality of their lives

Combining personalized, high-touch interventions with a pragmatic, cost effective staffing model reflective of the day-to-day psychosocial factors triggering many ER and hospital admissions, Integra ServiceConnect:

• Complements, rather than competes with, care management programs already in place
• Employs local Service Coordinators supervised by licensed clinicians, available 24/7 and interacting regularly with members in their homes, neighborhoods and treatment facilities

Under Integra ServiceConnect we do not attempt to treat members into better health service utilization. We help them avoid situations that can trigger admissions while facilitating access to more appropriate levels of care. Our focus is on identifying and modifying day-to-day psychosocial factors that encourage over-reliance on emergency rooms and hospitals when care is sought. In addition to working closely with members, Service Coordinators actively enlist the support of friends, family, clergy and health plan staff at every step along the way. Equipped with Tablet-based remote communications technology, our team is fully functional in the field, supported by real-time reporting, resource consultation and event tracking using a state-of-the-art web-based electronic health record.

Integra ServiceConnect also offers an alternative to Fee-for-Service compensation that incentivizes maximum billing and is common in traditional provider-based case management. Reflecting our deep managed care history and experience, we align our incentives directly with our clients' own financial and performance objectives, including risk/reward sharing arrangements under which performance and outcomes (e.g., HEDIS) determine our ultimate compensation.
